libgomp: move data definitions from icv.c back to env.c

* config/nvptx/env.c: Delete.
	* icv.c: Move definitions of ICV variables back ...
	* env.c: ...here.  Do not compile environment-related functionality if
	LIBGOMP_OFFLOADED_ONLY is set.
