This commit brings all the changes made by running gdb/copyright.py
as per GDB's Start of New Year Procedure.
For the avoidance of doubt, all changes in this commits were
performed by the script.
I was looking at some "set debug lin-lwp" logs, and saw that a thread
received the "Child exited" signal. It took me a moment to realize that
this was SIGCHLD. I then thought that it would be nice for
status_to_str to show the signal name (SIGCHLD) in addition to the
description "Child exited", since people are much more used to referring
to signals using their names.
Fortunately, libiberty contains a handy function to get the signal name
from the signal number, strsigno, use that.
The output of "set debug lin-lwp" now looks like:

Instead of using a static buffer. This is safer, and we don't really
mind about any extra dynamic allocation here, since it's only used for
debug purposes.

This eliminates the need to specify the return type when using
handle_eintr. We let the compiler deduce it for us.
Also, use lowercase for function parameter names. Uppercase should
only be used on template parameters.

The debugging code in linux-waitpid.c is one of the few remaining
spots that depends on the gdb/gdbserver difference.
My first thought was that this code is not extremely useful, so this
patch removes this code. (However, if it is actually useful to
someone, we could make it work by introducing a new abstraction.)

As the file was copied from GCC's libstdc++-v3 testsuite, this commit
leaves this file untouched for the time being; a patch to fix the header
was sent to gcc-patches first.

This applies the second part of GDB's End of Year Procedure, which
updates the copyright year range in all of GDB's files.

Since we now rely on PTRACE_EVENT_CLONE being available (added in
Linux 2.5.46), we're relying on NPTL.
This commit removes the support for older LinuxThreads, as well as the
workarounds for vendor 2.4 kernels with NPTL backported.
- Rely on tkill being available.
- Assume gdb doesn't get cancel signals.
- Remove code that checks the LinuxThreads restart and cancel signals
in the inferior.
- Assume that __WALL is available.
- Assume that non-leader threads report WIFEXITED.
- Thus, no longer need to send signal 0 to check whether threads are
still alive.
- Update comments throughout.
Tested on x86_64 Fedora 20, native and gdbserver.

* linux-low.c (linux_write_memory): Rewrite debug output to avoid
reading beyond the passed in buffer length.
This commit makes linux-waitpid.c include common-defs.h. GDB's
inclusion of defs.h is removed, but gdbserver's inclusion of
server.h remains to support some gdbserver-specific debug code
that cannot presently be merged. A new FIXME documents this.
