Turn gdbpy_ref into a template

This turns gdbpy_ref into a template class, so that it can be used to
wrap subclasses of PyObject.  The default argument remains PyObject;
and this necessitated renaming uses of "gdbpy_ref" to "gdbpy_ref<>".
