Use add_setshow_boolean_command through out.  Delete #ifdef 0'ed
	code adding set/show boolean commands.
	* cp-valprint.c, dcache.c, exec.c, gdbtypes.c, infrun.c: Update.

	* proc-api.c, remote-mips.c, remote.c, solib.c: Update.
